Incident Report Form
Use this form to share information about student behavior, including hazing, bias, or other conduct-related behavior. The behavior may or may not violate the 海角社区 Honor Code. Anyone who becomes aware of an incident or relevant information is encouraged to report it promptly. 海角社区 University staff will review the report and coordinate a resolution, which may involve the Office of 海角社区 Standards & Support, Office of the Dean of Campus Life, Residence Life, Title IX and Sexual Misconduct, Inclusive Excellence, or other appropriate offices.
CARE Report Form
Use this form to share significant concerns about a student鈥檚 well-being, including social, financial, family, or mental health issues.
Harassment & Discrimination Reporting Form
Use this form to share any experience of discrimination or harassment. This includes, but is not limited to, sexual harassment (including sexual assault, dating violence, domestic violence, and stalking) and discrimination based on race, color, national origin (including shared ancestry or ethnic characteristics), sex (including sexual orientation, gender identity, and pregnancy-related conditions), age, disability, genetic information, religion, veteran status, or any other protected basis under applicable law. Compliance and Title IX staff are responsible for the initial review of these reports.

Use this form to report academic violations that take place in courses only.
